Position Description:
JJPI's Survivor Services (SS) Outpatient Program provides high quality treatment services to individuals who have experienced one or more traumas impacting their mental health.
Services include individual therapy, family therapy, group therapy, and psychiatry. The program offers both in-person and telehealth services, although all intake appointments are on site.
-Work with JJPI's clinical and administrative team to support program participants in their recovery/treatment journey.
-Meet with participants as part of JJPI's intake process and provide support/coordinate addressing social determinants of health
-Collaborate with JJPI's clinicians/therapists, intake/case management team, administrative staff, and supervisors regarding non-clinical participant support
Position Requirements:
We are looking for individuals who have participated in mental health treatment and have overcome or are in the process of overcoming emotional/mental health issues. Individuals who have experienced trauma and have received mental health trauma treatment preferred.

Nutrition Education CHW
New Kensington Community Development Corporation
Position in Philadelphia, Pennsylvania
Position Description:
New Kensington Community Development Corporation's Nutrition Education team is looking to bring on a community health worker to assist in facilitating classes and workshops as well as the organization's community outreach efforts. The CHW will work with the Nutrition Education Managers to help facilitate nutrition education programming, build relationships with community-based food distributors and partner organizations and support Policy, systems and environmental changes in Kensington. Programming entails providing hands-on or web-based lessons and workshops while addressing specific areas of expertise such as nutrition, healthy cooking, exercise, food budgeting and goal setting using our standardized curriculum. Additional training and guidance will be provided on the job, and there is an option to complete a certificate in nutrition as well.
Position Requirements:
Applicant must be comfortable standing for extended periods during events and will likely be asked to occasionally assist in setup that requires lifting as much as 20 pounds at a time.
Applicant should be comfortable interacting with many members of the community as well as with public speaking.

Community Health Worker
Latino Connection

Position Description:
Latino Connection is a public health nonprofit providing health outreach and education resources to underserved communities. The dedicated staff at Latino Connection intimately understand the unique values, motivators, and concerns of Latino communities and work to empower and uplift the most vulnerable – and wonderfully vibrant- communities. They offer a variety of services, ranging from mobile COVID testing vans to nationwide webinars and community outreach events to raise awareness on menthol tobacco. They are seeking two already trained Community Health Workers (CHWs) to assist in some of their current programs.
The CHWs will support the Blood Pressure Self-Monitoring Program (BPSMP). They will play a vital role in helping participants manage their blood pressure by providing guidance, education, and ongoing support. This position involves working closely with participants to encourage lifestyle changes, track progress, and connect them with necessary resources.
Position Requirements:
High school diploma or equivalent; additional education in health-related fields is a plus.
Certification as a Community Health Worker is preferred.

Peer Recovery Specialist (2 open positions)

Position Description:
The Recovery Support Services program, an enhancement service for the New Pathways Project, supports individuals in recovery from substance use disorders. The Recovery Support Services program provides non-clinical services that assist individuals in recovery in gaining the skills and resources needed to initiate, maintain, and sustain long-term recovery. The RSS program offers services such as care coordination, recovery coaching, group support, supported employment/training, transportation, and assistance with accessing recovery housing. Recovery support services are not a substitute for clinical services. Recovery support programs are person-centered and self-directed, allowing the individual in recovery the choice of provider.
The Peer Recovery Specialist provides recovery-oriented support services to individuals experiencing substance use and/or mental health challenges. Utilizing lived experience, the Peer Recovery Specialist fosters trust, promotes empowerment, provides emotional support and encouragement, helps individuals build coping skills for maintaining sobriety, and assists individuals in navigating treatment, resources, and community support systems. This role involves collaboration with clinical teams, if necessary, advocacy, and engagement with individuals to support their recovery journey.
Position Requirements:
High school diploma or GED
Lived experience in recovery from substance use, with a sustained recovery period
Certification as a Recovery Specialist (CRS) in Pennsylvania, or willingness to become certified within 90 days of employment
Knowledge of community resources and recovery support services

Peer Recovery Specialist

Position Description:
Pathways to Recovery (PTR) is Philadelphia’s short-term co-occurring disorder treatment partial hospitalization program. The program is funded by Community Behavioral Health (CBH) and Behavioral Health Special Initiative (BHSI) for Philadelphia residents with co-occurring substance use and mental health challenges. Our program works with participants 20 hours per week, for up to nine weeks. We provide a menu of treatment services available each day including individual, group, and family therapy; medication-assisted treatment and medication management with our psychiatrist; health care coordination and full-time nursing support; full-time case management; and full-time peer recovery specialist support.
We are seeking a Certified Peer Specialist (CPS) to join the Pathways to Recovery (PTR) team as a Peer Recovery Specialist. This individual will use their own lived experience to engage and support PTR participants.

Certified Peer Specialist (CPS) certification
High school diploma or GED
1+ years of experience working or volunteering with individuals struggling with addiction and mental illness
Valid driver’s license in good standing
Ability to drive a large van

Community Health Worker/Specialized Health Services
Public Health Management Corporation (PHMC)

Position Description:
A Community Health Worker is sought to support a Complex Care Management Team through an integrated approach to care management and community outreach. The Community Health Worker will report to the Sr. Director of Behavioral Health Integration and the Director of Social Services and will be responsible for the identification and engagement of high-risk Health Network patients in the community. The position will be responsible for connecting identified patients to the Complex Care Management Team and to initiating targeted outreach in Philadelphia to patients that have had a lapse in care, especially persons with specified chronic conditions.
The Community Health Worker will also work closely with staff across six PHMC FQHC sites and with stakeholders in the community to engage and link patients to Heath Network services. The position will additionally be responsible for assisting patients in understanding and advocating for their health needs, and in navigating and accessing health and social services relevant to their primary care. The full-time Community Health Worker position will require frequent, local travel and may include home visits and program screenings, where appropriate. It may also include escorting patients to appointments, as necessary.
Position Requirements:
Excellent time management skills and attention to detail to attend to program target goals and to ensure timely documentation
At least two years of experience with vulnerable populations, including incarcerated, substance use and/or mental health disorders; knowledge of Philadelphia is a plus
Ability to adapt to fast paced medical environments and demonstrate flexibility in operating within different work and community settings
Knowledge of medical (especially chronic diseases) and behavioral health conditions
Excellent interpersonal, organizational, and oral/written communication skills
Ability to represent the agency and facilitate a relationship with the community and other providers
Ability to maintain client confidentiality
Written and oral fluency in Spanish and English preferred
